Tom McGowan, mentioned by SidL elsewhere in the thread, went on for Ron Silver at the first preview of La Bete in Boston. Then Silver played one performance and refused to go on again. If we accept the idea, so strongly stated elsewhere in the thread, that understudies do not get a rehearsal till after the opening (although I know of at least on exception to that), then McGowan went on with no rehearsal in a massive role.

Christine Andreas went on for Teresa Stratas on opening night of Rags in Boston. She says that she'd never has a rehearsal. (See linked article from just a few years later.)
