Sitting on house left, even I was briefly puzzled when Colonel Ricci said to Fosca "That was beautiful, my dear," after Torasso finished singing "La pace sulla terra." All I could see, even from my basically excellent seat, was Fosca's back. With no piano there, and nothing representing a piano, and her back to me, I had no idea for a few seconds what she had done that was beautiful. Then I remembered, but I'd guess that some people were truly puzzled, even if they then forgot about it.
