At The Old Globe this past summer, it was initially difficult to hear a number of the lines, as the stage speech did not sound amplified. I thought that this problem was mostly the cast and audience getting used to each other in the space, and it did seem to right itself. I think that the musical numbers are amplified. There was a sound designer credited.

Also, I noticed that cast members created sound effects from the sidelines, and there were mics present that were used specifically for those effects.
