The Playbill is not out yet, but from the way that the actors' names are displayed on the official website, I believe that all four actors in Meteor Shower are above the title.
It's pretty unusual for an above-the-title actor to be replaced/fired in rehearsals or previews. (I am not counting examples where someone was replaced after the out-of-town tryout.) Shia LeBoeuf left Orphans in rehearsals, replaced by Ben Foster. Although the production closed down, this essentially happened to Faye Dunaway in the Los Angeles production of Sunset Boulevard.
