There was someone involved in the original Les Miz (and still active in other aspects of the biz) who has been often been whispered about, but I don't know that this person has ever been publicly called out. (But I can assume that many of you will know exactly who I'm talking about.)

And this didn't happen on Broadway, but sure involves someone who has performed there a number of times - some of you may have heard about the incident involving Tom Wopat in a Boston area 42nd Street this summer, who was brought up on charges for groping women in the cast, and was also accused of other irresponsible behavior (being drunk in rehearsals, coarse language, etc). He was fired on the day of the first performance. In this case, the woman involved came forward right away, and thank god she had the courage to do so. We do have heroes.
