I was at the preview of Shogun when a fly dropped and hit Philip Casnoff. Thank God he turned out to be okay but he was lying on the stage and not moving for a very long time. They asked the audience to leave. Very scary.

The mishap I didn't see but wish I had was a performance of My Fair Lady. Instead of raising a chandelier it was lowered. They finally managed to raise it, taking Rex Harrison's toupee (which got caught) along with it.
