Tewfiq is so damaged by what happened to his family, which he blames himself for, that he doesn't see himself as deserving of any happiness. He is suffocated by his guilt.
It's been a while since I've seen the show, so my memory may be faulty, but I seem to remember that he was a bad or distant father and his son committed suicide, devastating Tewfiq's wife.
