They're pretty unpleasant--from either side of the cigarette. Equity has approved brands/versions of e-cigarettes (including non-nicotine options) to protect the actors, and they do check to confirm that the show is using an approved version. I'm not aware of any codified policy on herbal cigarettes, though. Most actors would rather use e-cigarettes when the staging permits it, but there is a certain lack of verisimilitude that may make them undesirable. (They don't burn down in the way that a real cigarette would if they are used over a period of minutes. And they clunk when they hit an ashtray.)

I hate e-cigarettes on stage. I saw an off-Broadway play and the female lead smoke one and it looked completely ridiculous. It had like a light at the end and she would pretend to inhale but nothing came out. Too distracting.
I also hate that herbal smell.

I prefer using fake cigarettes which when used correctly allow you to emit a sort of powdered smoke. They do have the same problem of not burning down but I find that a fair trade off for the health and safety of the actors and audiences.

The problem with those is that you blow the puff of "smoke" when the cigarette is in your mouth, and then when you take the cigarette away, there's nothing. Which is not how cigarettes work.

They make very realistic e-cigarettes that only light up when you inhale, just like an actual cigarette. It does have the problem of not burning down and yes, they do make that unfortunate "clunk" in ashtrays if you don't set them down ever-so-gingerly. But I prefer that to puff cigarettes and especially to the noxious herbal ones.

I used e-cigs for a while. Someone makes one like you describe and it is also has a "soft" filter tip so that it does not make as pronounced a clunk sound as others. I forget, of course, the brand name. One of the, albeit minor, concerns I have with actors smoking is that most have never really smoked and do not know how to hold the cigarette realistically and also do not know how to fake inhalation. Consequently they simply draw the smoke into their mouths and then blow it out. (no Bette Davis nose exhalation...sigh) Well I said it was minor...maybe even silly but I do notice it for some reason.
