When someone in a show has billing, standalone, as "And" after the first-billed stars, are they effectively being given 2nd billing?

For instance, "Annie" had starring "Andrea McArdle Reid Shelton" followed by "Sandy Faison Robert Fitch" on second line in smaller type or font, then "And Dorothy Loudon" in type similar to McArdle and Shelton. Was this kind of billing worked out somewhere along the line to give a "name performer" a show what seemed like a singular billing, but was still considered to be the second billed star of a show? It'd be interesting to hear your takes on this. Sometimes they put the "and" person's name in a box. Does this effect whether they are up for Lead or Featured for the Tonys and other awards? I have a feeling there has been much variance in this over the years. Of course, when producers won't budge on salaries, billing is very negotiable (and cheaper!).

Also, usually the left-billed person is the bigger star, followed by the person on the same line to the right. However, in "Wicked" you had left billed "Kristin Chenoweth" with right-billed "Idina Menzel" on the line above her. I seem to remember this in films years ago in things like the "Towering Inferno" when Paul Newman and Steve McQueen seemed to have similar billing. There seem to have been a lot of creative ways to give people billing over the years!
