Not sure about your question specifically in theater billing, but for television, the "and" credit is considered higher and comes with higher minimum wages.
Speaking of creative billing, I remember back in the day when Bette Midler and Shelley Long costarred in OUTRAGEOUS FORTUNE, Long got top billing in advertising west of the Mississippi and Midler top billing east of it. |
