re: When someone in a show has billing, standalone, as "And" after the first-billed stars, are they effectively being given 2nd billing?

Common sense is a good idea, but I'm not sure it always works.

For example, Lotte Lenya was considered a lead in the same role in which Mary Louise Wilson was considered supporting in Cabaret.

And Joel Grey was considered supporting in the same role in which Alan Cumming was considered a lead, also in Cabaret.

I could make a case that the band leader role in The Band's Visit is a supporting role, but it's considered a lead.

And of course the king in the King and I has been considered both a lead and a supporting role.

One person's common sense is not the same as the next person's. Ask Rita Moreno (The Ritz) and William Daniels (1776).
