I can't speak to Broadway, but it's definitely not the first time a real baby played a real baby in a play. I saw the first preview of Diane Paulus's The Gershwin's Porgy and Bess out of town in Cambridge, MA, and they used a real baby. (I found it extremely distracting - I was too worried about whether the baby would cry to pay attention to "Summertime.") I believe by the time the show came to NY, the real baby was replaced by a doll.
