I don't wish to wade into this discussion except to mention something about one point you made: "Bennett's conceit has never been repeated (to my knowledge); if anything, it's been strategically avoided."
Several of the major regional productions of the 1980s and 1990s borrowed the basic concept. Bennett was not generally (or perhaps not ever) credited, but the basics just as you described them were employed.
