| For the 1st dose, I got the following: CDC COVID-19 Vaccination Record Card It notes the date of the first vaccination w/ the healthcare person's initials. I must bring the card with me when I get the second dose in 2 weeks. It's an official card printed on 10PT SBS (I'm a former paper salesman), but it can be easily forged. The Tribune article says that the proposed vaccine passport will be made via "various technologies, schemes, and entrepreneurial ideas". |
