The reason for giving the card at the first dose is because not everyone goes back to the same place, and not everyone got the same brand. Likewise, not everyone goes back and yet the first dose provides more than "half" of the effectiveness of the vaccine.

There is a "clear piece of proof given for each dose." The actual proof is retained by the giver of the shots, and in due course there will be a reliable system for proof but you wouldn't want them to wait to dispense until they have all the details worked out, would you?
