I don't think there is a hard and fast rule about whether the standby has to be in attendance in the building, and different productions have different rules. In some cases, the standby has to be in the building up until at least a certain point in the show (sometimes, the character's final entrance). In some cases, the standby doesn't have to be physically in the building the whole time but has to be within a certain radius. But I would think in general, it's the standby's job to go on if the "regular" person can't, even if it's mid-show or last minute.

But that doesn't mean it actually happens that way. I mentioned earlier that I saw MJ recently, and the "middle Michael" was out at the last minute. There is a "middle Michael" standby, but an understudy went on, not the standby. I don't think "middle Michael" appears right away, but the opening number of the show has intensive dancing by "adult Michael/MJ." I could see a scenario where, rather than waiting for the adult Michael standby to get warmed up, they would just have the understudy do the role, since that person is ready to go (albeit in a different role).
