I think you may be confusing an actor of note with someone who is famous. Stand-bys are rarely famous. I just went through some programs looked to see who was listed as standbys and I saw Maureen Anderman, Joan MacIntosh, Stephen DeRosa, and Michael McGrath.

This was a very unscientific sampling of about 10 programs. Only five of the shows had standbys (Anderman was standby for two) and as you can see all were actors of note. I am sure at some point there have been standbys whose names would not be as familiar and who might not have as impressive resumes. But I am guessing that is pretty uncommon.

I know musicals do things differently, because in musicals understudies are usually members of the ensemble. So I have a feeling everything is different there.
