Here are some examples (sort of, but not always exactly the same as yours):

Lotte Lenya originated the role of Jenny in the original 1928 stage production of The Threepenny Opera, did the film in 1931, and played the role again in the famed 1954 off-Broadway production.

Debbie Reynolds played the title character in the 1964 film of The Unsinkable Molly Brown and then played the same role on stage in the 1980's.

Giorgio Tozzi sang the role of Emile in the 1958 film of South Pacific, then played the same role on stage for years.

Jane Powell and Howard Keel starred in the 1954 film musical Seven Brides for Seven Brothers, then many years later played the same roles in a stage version of the show.

Jeanette MacDonald played Sarah Millick in the 1940 film version of Bitter Sweet, then played the same role on stage in the 1950's.
