Yeah, many times. Not in NYC admittedly, but for shows that have finite runs and then get extended or sell better than they were projected to, quite often they run out of programmes and decide not to print more. Recently they give you the code to capture with your phone and sometimes they'll give a print-out of a cast list.

I've also seen shows in London where the programmes- which cost anywhere from £5 to £20-have been completely sold.
